Documentation Requirements

Complete passport application with the exception of signature. Singnature must be in person in front of a deputy clerk.

You need your original birth certificate from the health department. You cannot use a physician or hospital certificate. You birth certificate will be returned to you with your passport.

You need two pictures of correct size, color, and payer weight. Just tell the photographer you need a passport photo and they will know the specifications.

You will need to bring photo id (drivers license.)

Applications for children under the age of 14 require both parents signature witnessed by our office.
